MA Card (Mukhyamantri Amrutam / MA Vatsalya Card)

The MA Card (Mukhyamantri Amrutam / MA Vatsalya Card) was the flagship health protection scheme launched by the Government of Gujarat. It has now been integrated with the central Ayushman Bharat Pradhan Mantri Jan Arogya Yojana to form the unified PMJAY-MA (Ayushman Card) in Gujarat.

Where to Apply & Get the Card

• Jan Seva Kendra (ATVT): Located at your local Taluka Mamlatdar Office or District Collectorate (Jilla Seva Sadan).

• Civil / Government Hospitals: Helpdesks and kiosks (Kiosk / Ayushman Mitra desk) at District Civil Hospitals, Sub-District Hospitals, and Community Health Centres (CHCs).

• Common Service Centres (CSC) / e-Gram: Local Gram Panchayat e-Gram centers across rural Gujarat.

• Online via Beneficiary Portal: PMJAY – Beneficiary Portal to verify status and download the updated digital card.
